I'm not sure I quite understand, so If what I suggest doesn't work you may want to hear what others or 'rabid/gordon' has to say.

So are you running R2 on a laptop w/built in sound and using the video out of your laptop to another monitor/Tv?

If thats the case then I assume NONE of the external visuals currently work (if you tried anyway). I remember there being a problem w/visuals detecting sound on some version, you may want to grab the latest Winamp 2.91 or try selecting the 'What U hear' box in your audio record properties also see whether you have 'Digital output only' in advanced volume properties checked or not. (That is if they apply to your OS)
